Application Process
Contact the LDN administrator, Anne Duifhuis, if you have questions about the program: office@crc1life.ca or 604-358-1310.
You must submit the following three forms by September 1 of the year you are applying. There is a $25.00 fee for applications received after that date.

The facilitator will contact you with further details when your application has been reviewed.

Keep a copy of your application on file. Please inform others filling out forms for your support/reference, where to send them.
Participant’s Application Form
- To be completed by you, the participant, with a copy saved in your files.
- To be completed by your pastor and sent directly to the facilitator.
Tuition Payment
May be paid in full ($1500) or with two cheques ($750 one for September and another post-dated for January, again for $750) made out to CRC Classis BCNW LDN.
Please bring the cheques with you to the first regular LDN meeting in September.
